I believe the aviptadil trial indicates that the reversal of ARDS is a slow and gradual process. Immune restoration with leronlimab (or aviptadil) does not rapidly reverse the damage. Stopping further inflammation allows a gradual recovery.
I believe this is why the avidtadil effects were more evident in days 28-60 than 0-28, and arguably why the DSMB apparently saw a sign of additional placebo mortality in days 28-42.
