I don’t know who you use or how often you check your portfolio. I use TD Ameritrade. Shortly after I got my “NGIO” dividend (still listed as Antigen Express), TD removed the stock from my position list because they said it has no assigned value. I had to contact them explaining what the “NGIO” dividend is and tell them to put it back in my stock list portfolio, which they did. Perhaps this is what happen to you with whatever broker you are using. The adding and removing could have happened without you knowing if you have large intervals between when you check your portfolio.
