You're exactly right. In fact, the guys at BioRunUp.com developed an entire trading strategy based on the ramp in share price that usually occurs before FDA decisions/PDUFA dates/etc.
And you're also right about traders not holding into the actual decision itself. Two rules of day trading carved into stone are never hold into earnings or FDA decisions, because - as you correctly stated - they can go either way.
That being said, I'm 2 for 2 holding tickers into favorable FDA decisions. One was PTLA, the other ticker escapes me. Something to do with cartilage regeneration or something like that.
Leronlimab for Covid is about to make me 3 for 3.
