Just so everyone is aware Doc submitted 4 forms this time last time it was only 3 forms...
1. CMS-855B: is to be used by Clinics/group practices and certain other suppliers.
2. CMS-855R: Reassignment of Medicare Benefits. This form is used by individual physicians/practitioners who are reassigning Medicare benefits to an enrolled group, organization, or individual or who are making a change to a previously established reassignment.
3. CMS-20134: Medicare Enrollment Application for Medicare Diabetes Prevention Program (MDPP) Suppliers.
4. EFT: Electronic Funds Transfer (EFT) Authorization Agreement.
