I guess I have an answer to both your questions.
Why so few shareholders?
Very often, at least here in Germany /Europe(??) vanks/brokerage firms officially represent all of their shareholders. Which means all GNBT /NGIO shareholders from Deutsche Bank would not be listed individually, but as one large Deutsche Bank shareholder, and DB would then manage every individual internally, if there is information or Divi shares to be distributed.

Why has that number changed?
It could be part of a deal with some of the cooperation partners in that vaccine development process that received NGIO shares instead of cash.
OR it might as well be that some international GNBT shareholders have contacted the TA and requested to receive their Divi shares, which were not automatically distributed to them. E.g. I had to write an email for my parents to state to the TA that I had not received the dividend automatically. It is possible that such individual shareholders would now be listed individually, which works explain why the number of shareholders went up.
