You dont have to time ... just keep them ... 
Maybe timing not that diffcult btw, what about 1-2 years before reaching peak sales in all indications, or if it reaches 5x on estimated peak sales of all indications.
... and Pharma is recession proof ... just look at them during the last meltdown, easy going.
And to hedge yourself against politics and other things, just insure your portfolio with Gilead and MRK puts, as I did.
Wouldnt be surprised if both sides of that trade play out really well
