I could be completely wrong, but your 2nd graph seemed the most understandable to me.
Here is my take:
Dose 3 patients at 350mg
a) if 0 toxic response, escalate dose to 525mg for 3 additional patients...then 700mg
b) if 1 toxic response, dose 3 more at 350mg....if 0 toxic response follow a.
c) if 2-3 toxic response...stop/failure
