Supplemental Information Disclosure for Supurva Healthcare Group, Inc. (the “Company”):
1. On January 15, 2018, Bryan Geer was elected as a Director and appointed President and
Chief Executive Officer.
2. On July 10, 2018, Bo Linton was elected as a Director of the Company and appointed
President and Chief Executive Officer, and Bryan Geer resigned as Director and Officer
of the Company.
3. On July 10, 2018, the Board of Directors and the Shareholders Required to Vote,
approved a plan to acquire 100% of Web To Door Corp.
4. On July 16, 2018, Rod Santulan was elected as a Director of the Company and appointed
President of the Company, replacing Mr. Linton.
5. As of August 1, 2018, the Company’s principal place of business is located at 60 – 98th
Ave. Suite 100, Oakland, CA 94603.
6. On September 14, 2018, Mr. Santulan replaced Mr. Linton as CEO of the Company upon
the resignation of Mr. Linton from the Board of Directors and all other positions he holds
with the Company
