The market makers simply do not show all of the bids and asks that come through them. Remember that there are FOUR primary exchanges for OTC stocks. A crooked market maker (sorry for the redundancy) can just not submit the data for the Level 2 data aggregator, and then wham the trade in when they want it to cross. It's so instantaneous that it never shows up on the Level 2.
