Haven't decided yet how to forward the completed form.
Interesting though, that there was no blank to enter the number of shares each SH was voting. That info, I believe, was included in proxies I've received in the past. Maybe the Company already holds the majority of outstanding shares... so that it matters not how many shares are about to be tallied. The results may already be decided. If that is the case, I don't mind at all ! GO $NTEK$ !!!
