My understanding for years has been that bids of 5,000 shares and above are the only bids required to be listed. As for bids and sales being filled, it matters whether bids and asks are listed as "all or none" or "any amount". You can try buying 200,000 shares at all or none and have it as good till canceled and you may never be filled unless you significantly bid up the price.
