While standards are great...it has turned into a white collar heavy bureaucracy rather than a health service delivery system.
Your copays provides the service. Your premiums feed the fat cats...insurance company's have predetermined what treatments cut into their bottom line in advance. Denying services maintains their cost and bottom line.
Where the disfuction is most insurances have such a high deductable it renders the insurance useless to most lower income people.
Cut out the fat cats that don't provide direct patient care. You might get a system that works and is affordable for all.
