You might also want to correct #54:
54. No shares have been added to the A/S after R/S
Attorney letter dated May 18, 2012 shows 36,702.286 shares outstanding.
Attorney letter dated August 21, 2012 shows 37,364,395 shares outstanding.
That means that 662,109 shares were added to the O/S somewhere between those two dates which would make this point incorrect. There is no evidence/DD to the contrary.
