Maybe true just for Wolframs. It could be difficult to maintain the concentration of MANF in the optic nerve. However, i believe this wouldn't be a problem with applications in the eye (RP and CRVO). For brain applications they plan to use a permanent delivery system that is currently being tested with GDNF. The delivery mechanism may be Urano's problem with MANF. Hopefully they can find a solution for this obstacle.
