The issue with the blood brain barrier (BBB) is intravenous MANF getting to dopaminergic cells in the brain. That is where the Renishaw catheter comes into play. It goes right to the source to deliver the goods. As for MANF and diabetes type 1, intravenous injection does not have this issue.
