I just did one, I didn't have to get a new 144 cert, the TA sent me a form in which there was a section for "If this was issued under a different name", I filled it out, got the Opinion Letter from the Corp attorney attesting to the facts that included the original transaction and subsequent name change and I received a new cert in the new name.
And I've been doing this since 1981. I've done more than a few dozen of these, I'm positive. Just looked at the TA site for my last one and they still list the form as the one to use for name changes.
