Sometime they're entered as private transactions as well.  Large buys/sells if seen on the open market can have a significant impact either way on an issue.  If I wanted to accumulate large blocks, you wouldn't see my bid on Level II.  They would either be private, or they would be AON to the ASK price, especially if I see VFIN on the ask.
