To be crystal clear, unfamiliar forms need to be filed with your broker of choice. This homework requires concentration which does NOT allow for mistakes. Expect further unforgiving delays if not done properly. This homework is not a joke. Just as we needed to follow directions from the 12.3.2013 press release, so too, the brokers expect flawless paperwork for a smooth transaction.
