IDCC Expands Community Support Through WholeYouNYC Initiative
Enhancing health services, navigation, and community connections through New York's Social Care Network

Its mission is to bridge the divide between healthcare and essential social services, thereby improving health equity for residents across New York. By placing individuals at the center of care, WholeYouNYC effectively connects them to resources, whether it’s food, housing assistance, employment support, legal aid, or health services, using a culturally responsive and navigator-led approach. The initiative collaborates with more than 300 community-based organizations, successfully facilitating access to coordinated services for thousands of residents in Brooklyn, Queens, and Manhattan.
IDCC Community Distribution Events
In December, IDCC took significant strides towards implementing this innovative model through their Winter Community Distribution events held on the 16th and 17th in Brooklyn. During these events, IDCC staff actively engaged with residents in need of essential winter supplies, while also connecting them seamlessly with WholeYouNYC navigators. These direct interactions created valuable opportunities for immediate needs and also initiated referrals into the Social Care Network, linking individuals and families to much-needed support such as housing assistance, nutritional help, behavioral health services, transportation aid, and more long-term care options. IDCC plans to maintain this outreach initiative, with additional community events scheduled throughout the winter months to further bolster SCN access.
